VOLUNTEER FORMS

Volunteers are required to register each year.

If you are a returning volunteer, follow these instructions to register in the system.
If you are a new volunteer, follow these steps to register in the system.

FAQ About Volunteer Forms:

  • All volunteers who come in contact with youth players or youth referees must fill in a Volunteer Form.  We crosscheck our database with the Volunteer Form database and if you have not completed the form you may not serve in a youth contact volunteer capacity (that is an AYSO mandate).
  • Youth Referees and Youth Coaches must complete a Youth Volunteer Form.
  • New Volunteer Forms are required each year--however after your first year you can update your Volunteer Form through eayso.com.
  • First time users will need to establish a username and password.
  • The Volunteer Form must be completely filled in. All references must be complete: Name, address and phone number. All appropriate boxes must be checked
  • Social Security Number and Driver's License (or appropriate, acceptable ID) number are required--we cannot accept your Volunteer Form without both.
  • If you are a new  volunteer, print three copies of the form, keep one for your records and mail TWO COPIES  of the form to:

SAFE HAVEN

What is Safe Haven?
Safe Haven is AYSO child protection program.
Who should take it:  

Any adult who comes in contact with AYSO players. That includes parents who want to participate with their children and teammates at practices.
Do I ever need to renew my Safe Haven?  

Maybe.The program gets updated from time-to-time. You will be notified if you need to re-take the web based safe haven. 
How do I verify that I took Safe Haven?  

Contact the Child & Volunteer Protection Advocate (CVPA). Click on the Contact Us link to find out who the current CVPA volunteer is.
When are classes available? If you are a new coach or referee, Safe Haven will be included with your classes, otherwise you may take it on line. Please note that in order to take the Safe Haven class you must have an AYSO ID number. If you don't know what your number is please contact the CVPA.
